Our goal is to provide the highest quality, family-centered medical care to newborns through adolescence and to achieve optimum health for children. We strive to be on the cusp of new healthcare quality initiatives which raise the standard of care for children and families. We believe that the more organized and streamline we can be, the easier it is for our providers to focus on the most important aspect of why we are here – our patients.
Meaningful use is the use of certified electronic health records (EHR) technology. By using this technology we can improve quality, patient safety, and help reduce health disparities. It can help us engage patients and families with the use of a patient portal. The use of EHRs can improve care coordination and help maintain the privacy and security of health information.
Patient Centered Medical Home (PCMH)
The medical home model holds promise as a way to improve health care in America by transforming how primary care is
organized and delivered. The primary care medical home provides primary health care that is relationship-based with an orientation toward the whole person.
Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences.
The medical home
practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core
members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans.
